Cord Blood Search Request and Release Process

The Cord Blood Donor Foundation’s laboratory and storage facility is located at the University of Arizona. Dr. David Harris, is the director for the cord blood laboratory. To initiate a search for a potential cord blood match please read the following guidelines:

To begin the search for a potential cord blood match, the physician or transplant center may contact the Cord Blood Donor Foundation to request a Preliminary Search Request Form. (You may also click here to retrieve the form).

After gathering all necessary information, this form must be faxed or sent to Dr. David Harris at the University of Arizona where a preliminary search will take place (instructions are on the form). Matches are performed for 4/6, 5/6 and 6/6. This service is free of charge.


The laboratory will notify the requesting physician of the results from the preliminary search.



If a potential match is found, the transplant center may request an aliquot sample for confirmatory testing by completing the Request for Aliquot Sample Form. All request forms are to go through Dr. David Harris at the CBDF Lab. There is no charge for the aliquot sample.



The transplant center may request the release and purchase of the cord blood sample once they have provided Dr. David Harris with a Request for Cord Blood Release Form and all confirmatory testing results.



Once all necessary information has been provided to Dr. David Harris, the transplant center will receive a purchase contract from the Cord Blood Donor Foundation for release of the cord blood sample.
